GENERAL RELEASE OF LIABILITY
HOLD HARMLESS, WAIVER AND INDEMNITY AGREEMENT

I, the undersigned, hereby acknowledge to Magnolia Stables (Kristin Lacy), that:

1. I am actively aware of the risk(s) of participating in a class/activity on a farm and working around animals, which includes horses, dogs, cats, and any other animal on Magnolia Stables (Kristin Lacy ) property.

2. I assume all foregoing risk of using Magnolia Stables (Kristin Lacy) property, and accept personal responsibility for damages from my use, including, but not limited to injury, disability, or death.

3. I release, waive, and discharge Magnolia Stables (Kristin Lacy), from any and all liability to me, my heirs, and next of kin for any and all claims, demands, losses, or damages, related to my use of Magnolia Stables (Kristin Lacy), property. I agree to accept full responsibility for any guest whom I bring to Magnolia Stables (Kristin Lacy) property, and will defend and indemnify Magnolia Stables (Kristin Lacy) against any claim brought by such person.

I have read the above waiver and release, understand that I give up substantial rights by signing it, and sign it voluntarily.

PHOTO RELEASE FORM

I, the undersigned, give Magnolia Stables permission to use my name, likeness, image, voice, and/or appearance as such may be embodied in any pictures, photos, video recordings, audiotapes, digital images, and the like, taken or made on behalf of Magnolia Stables.  I agree that Magnolia Stables have complete ownership of such pictures, etc., including the entire copyright, and may use them for any purpose consistent with the Magnolia Stables mission.  These uses include, but are not limited to illustrations, bulletins, exhibitions, videotapes, reprints, reproductions, publications, advertisements, and any promotional or educational materials in any medium now known or later developed, including the Internet.  

I acknowledge that I will not receive any compensation, etc for the use of such pictures, etc., and hereby release Magnolia Stables and its agents and assigns from any and all claims which arise out of or are in any way connected with such use.

I have read and understood this consent and release.

MINOR (CHILD) PHOTO RELEASE FORM

I, the undersigned, am the parent or legal guardian of any minors accompanying me, grant Magnolia Stables permission to use accompanying minors’ name, likeness, image, voice, and/or appearance as such in any pictures, photos, video recordings, audiotapes, digital images, and the like, taken or made on behalf of Magnolia Stables.  I agree that Magnolia Stables have complete ownership of such pictures, etc., including the entire copyright, and may use them for any purpose consistent with Magnolia Stables' mission.  These uses include, but are not limited to illustrations, bulletins, exhibitions, videotapes, reprints, reproductions, publications, advertisements, and any promotional or educational materials in any medium now known or later developed, including the Internet.
